Not too long ago, my father decided it would be helpful to his family if he wrote down his preferences and suggestions for the various details surrounding a funeral and burial. Not so much a "you must do it his way" document, but rather a way to take some of the burden off of those left to handle the arrangements. Answers to questions like "How would Dad have wanted us to to do this?"
He put his thoughts in a white three ring binder. He included pages for details like what church, music he liked, suggested pall bearers, details about his life we may want to include in an obituary.
He also included details for after the funeral was over. Bank account numbers, life insurance numbers, people in the family we might wish to contact. All items that will be needed before a will is opened.
He died recently, and his white book was an immense help as our family coped with loosing him, prepared his funeral, and dealt with the details of his estate.
After we had been through the process of saying good bye to my Dad, I took his white book, and added those details that we had encountered that he hadn't included, and made up copies for all his siblings and children.
